Send Patient a Feedback Survey

Use this form to manually send a patient a service survey email. Do NOT fill this out if they were booked with the above appointment scheduler and made their appointment.

 

Add Patient to Recall Campaign

Entering a patient's information here will put them in the Recall Campaign. They will be sent an email prompting them to re-book 12 months from the time they are entered here. You will be emailed 1 month after that, prompting you to follow up with the patient if they did not re-book. IF YOU WANT THEM TO BE SENT THE RECALL EMAIL ON AN EARLIER DATE, ENTER THE DATE IN THE LAST FIELD, OTHERWISE LEAVE IT BLANK!
